Microsurgery with the use of surgical microscopes was developed initially in the 1960’s to facilitate the repair of tiny blood vessels, using an operating microscope and surgical techniques already in use for middle ear surgery. It soon found wide application, especially in hand surgery and reconstructive plastic surgery.

Historically the some branches of surgery such as ophthalmology and otorhinolaryngology started to miniaturize early and start using microsurgery with the use of surgical microscopes. In the last quarter of 20th century, miniaturization started to develop most branches of surgery including neurosurgery and cataract surgery. The basic feature was minimization of trauma to the body tissues during surgery. Trends were small incisions, laparoscopic surgery by fiberoptic visualization through tubular devices, vascular surgery by catheters and microsurgery under operating or surgical microscopes to refine the procedures and reduce trauma. Many of the devices such as robotics and implants, aside from the surgical microscope, will be a part of this miniaturization process.

Micro ear surgery is done with a surgical microscope. Surgery for the human sensory organs like the ear requires precision to achieve desirable results.

The unaided eye is not adequate to visualize and work in the narrow confines of the ear where dimensions are measured in millimeters. Holmgren was the first among the many surgeons who pioneered the development of the microsurgery for the ear using surgical microscopes. He used magnification spectacle loupes with the surgical microscope to perform fenestration operations in the 1930’s for the disease of otosclerosis. The American Surgeon Shanbaugh in 1940 converted a dissection surgical microscope by adding a light source for illumination in the surgical microscope thus opening a new vista in reconstructive ear surgery.

Surgical microscope is a binocular microscope used to visualize fine structures within the area of a surgical procedure. It is also called operating microscope. It has wide range of surgical microscopes for different microsurgical procedures. Every Microscope has a pair of eye-piece and a separate set of objective lenses like Zoom and Step type. With an independent light path for each eye, you can see the object in “stereo” or 3-D (three dimensions). Due to the different ways of fixation, working distance and magnification, surgical microscopes instruments has different purpose that can be assembled with different parts.

If a stone lodged in a patient’s higher urinary tract, a fine type of endoscope is inserted through the bladder up to the ureter. The Uteroscope is a thin elongated fiber-optic device with small basket on its tip, used by the urologist to remove stones from the urinary tract. A flexible fiber laser uteroscope is used to break larger stones to smaller pieces to be carried out of the body through the excretion of urine from the bladder. (more…)
